The Maldives has the shortest journey of any market we serve — Malé to Trivandrum is a little over an hour — and the deepest structural reliance on South Indian hospitals. With a small population spread across many islands, the country provides excellent primary and emergency care but cannot sustain every tertiary specialty domestically. Overseas referral is not a workaround here; it is a normal part of how care works.

That shapes what patients need from us. Maldivian enquiries are more often urgent or semi-urgent than elsewhere in our markets, and the useful thing is speed and clarity rather than persuasion. A one-hour flight also means the calculation is different: coming back for follow-up is genuinely practical in a way it is not from Nairobi or Riyadh.

The Indian medical visa, from the Maldives

India issues an e-Medical Visa for treatment and an e-Medical Attendant Visa for family travelling with the patient, both applied for online in advance.

Eligibility follows your passport nationality. Maldivian nationals apply as Maldivian nationals; expatriate residents apply according to their own passport, and if that passport sits outside India's e-visa scheme the route is a paper medical visa at the Indian mission instead. We confirm which applies before you start.

You will generally need a passport valid at least six months beyond arrival with two blank pages, a recent photograph, a scan of the biodata page, and a letter from the treating hospital in India. We obtain that letter, which requires a reviewed case and a proposed date.

For genuinely urgent cases, say so at first contact. The sequence can be compressed when a specialist has seen the reports and agrees the case is time-sensitive, but it cannot be compressed retrospectively. Telling us on day one that this is urgent changes how we work it; telling us on day five does not recover the days.

Getting there

Malé to Trivandrum is typically around an hour and ten minutes, and Kochi is a short hop further. That is short enough that patients who would struggle with a long flight can usually travel, and short enough that a family member can come and go during a longer admission rather than committing to the entire stay.

It also makes staged treatment realistic. Where a plan involves an assessment, then a procedure a few weeks later, doing that as two short trips is often easier than one long one — and we will say so if it applies to your case.

Funding and insurance

Maldivian patients often have some form of coverage, whether through the national scheme or private insurance, and the terms for overseas treatment vary and are periodically revised. Check your current entitlement directly with your insurer or the scheme before you travel — we are not the authority on it and we will not pretend otherwise. What we can do is make sure the written estimate contains the detail an insurer or scheme will ask for, in the form they want it, which removes the most common cause of a claim stalling.

Where treatment is self-funded, you pay the hospital directly in Indian rupees at the hospital's own rates. We never handle your money and never add a margin. Our fee comes from the partner hospital for the referral.

What a trip actually looks like

You send reports and scans. A specialist reviews them and gives an opinion. We put it in writing: hospital, consultant and their medical council registration number, the procedure, a cost range and how many days to allow. Nothing is booked and you owe nothing.

If you proceed, we obtain the hospital letter, you apply for the visa, and dates are confirmed. You will usually arrive a couple of days early for pre-operative assessment, and occasionally that assessment changes the plan — which is the system working, not failing.

Bringing someone with you

The e-Medical Attendant Visa allows close family to travel with the patient, normally up to two people, applied for alongside the patient's application. Given the short flight, families from the Maldives more often rotate attendants across a longer admission than send everyone for the whole period. We can arrange accommodation on that basis if it suits you better.

Travelling from an outer atoll

Most guidance about medical travel from the Maldives quietly assumes you live in Malé. If you do not, there is a leg before the leg: a domestic flight or ferry to Velana International first, and that connection is the part most likely to go wrong. Weather affects seaplane and ferry schedules more than it affects the international sector, and a missed domestic connection can cost you an appointment slot at the other end.

Tell us where you are actually travelling from when you first make contact, not just that you are in the Maldives. Where a patient is coming from an outer atoll we build a night in Malé into the plan by default rather than assuming a same-day connection will hold. It costs one hotel night and removes the single most common cause of a missed pre-operative assessment.

It also affects the return. Being discharged and cleared to fly is not the same as being home, and for a patient still recovering, an unplanned overnight in Malé because the last domestic flight has gone is worth avoiding. We plan the return with that leg included.

After you go home

You leave with a discharge summary, imaging, prescriptions and a follow-up schedule. The short flight means in-person follow-up with the same consultant is realistic where it is clinically worthwhile, and we arrange it. Where review can be done remotely we set that up instead.
